ADDITIONAL COMMENTS: 1) This Facility is a closed Scrap Tire Monofill and a closed Municipal Solid Waste Landfill. 2) This Facility is secured by means of a gate at the facility entrance. 3) The Facility access road is of all-weather construction and is being maintained in overall good condition. 4) The Facility Surface and Groundwater monitoring is being conducted as required. The monitoring log, which includes the most recent surface water and groundwater sampling reports, was in order and presented for review during this Facility Compliance Inspection. The most recent sampling event was conducted on 19 September 2016 by Environment 1, Inc of Greenville, NC. The report was compiled and presented on 31 October 2016 by Smith Gardner Engineers, Inc of Raleigh, NC. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 9402_INSP_20170124 Page 2 of 4 5) The groundwater monitoring wells were observed during this Facility Compliance Inspection. All the wells were readily accessible, painted with high visibility paint, properly labeled, and secured with functional locks. 6) Permanent edge-of-waste markers have been installed as required and are being maintained around the toe of the landfill disposal area. 7) A cover-crop of native warm season grasses has been established and overall is well maintained throughout the landfill. 8) The cover crop is mowed throughout the year as needed. Portions of the cover crop have recently been mowed. 9) Some small trees and other woody vegetation were observed growing on the southwest outside slope adjacent to the old borrow area (area marked in red on photo below). *Please work to remove all woody vegetation from the outside slopes of the landfill. All outside slopes should be maintained free of brush and woody vegetation, graded at a slope not greater than 3:1 and seeded and mulched to establish a groundcover suitable to prevent erosion of your landfill cap system. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 9402_INSP_20170124 Page 3 of 4 10) Facility personnel have repaired areas of differential settlement in various areas around the landfill. *Please continue to monitor the landfill for areas of differential settlement and make repairs to these areas as needed. 11) Several small areas of ponding/standing water were observed on the top lift of the landfill. *As per our conversation, as soon as conditions enable repair activities to continue, please continue working to fill and grade these areas to promote positive drainage of surface waters away from the landfill. 12) Scrap concrete, concrete block, and brick is staged on-site in an area adjacent to the landfill for future grinding/crushing and re-use. The majority of the staged materials have been transported to the Washington County C&D Landfill, permit #94-04 and re-used in roadbuilding activities. *Please note that the State rules regarding recovered materials state that for a material to qualify as a recovered material, the majority of the FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 9402_INSP_20170124 Page 4 of 4 material (75% or more) must be sold, used, or re-used within twelve months of staging at the site. If the required volume of materials is not re-used within twelve months, then the materials must be properly disposed at a Facility permitted to receive such wastes. 13) *Corrective measures are required as a result of this Facility Compliance Inspection.
