OBSERVED VIOLATIONS:
1. .0844(b) All permitted septage management firms shall display decals or lettering on each side of every pumper
vehicle operated by the firm. The decals or lettering shall include the name, town name, phone number and
permit number. All decals or lettering on the pumper vehicle shall be no less than three inches.
Inspected replacement vehicle tag#FJ9570 - the permit number NCS-01268 is less than three inches. Correct
the permit number on both sides of the vehicle to at least three inches by February 24th and submit a photo
for each side of the vehicle to this office. At the time of this inspection on 2/10/2017 Mr. Shingleton was
instructed to correct the lettering. This vehicle has been in operation while being in violation of Rule
.0844(b).

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected new vehicle – tag#FJ9570, VIN 1FDUF4GT9CEC62559, 700 gallons. The tank appears structurally
sound and securely attached.
2. Outlet covers were on and no drips/leaks were noted.
3. The lettering is correct except for the permit number as noted on the observed violation on the front page of this
audit. Please correct the violation immediately.
4. Truck logs were not reviewed at this time.
5. This firm is permitted for the operation of two vehicles.
