(HRP) has completed a Limited Phase II Environmental Site Assessment (ESA) at the property located at 527 Atando Avenue, Charlotte, North Carolina (herein referred to as the Site). A Site location map is included as Figure 1. Please note that this ESA was performed at your request as a “limited” assessment in order to further assess the extent of chlorinated solvents in groundwater at the Site (MW-5, F&R report dated June 2013) and to evaluate whether the source of contamination exists on-site, or is a result of an unidentified off-site source. Of note, a 2011 Phase I ESA indicated a former industrial laundry service historically operated to the northwest of the Site. This ESA included the advancement of three (3) soil borings at the Site to further assess the above-noted concern. Each soil boring was converted to a permanent groundwater monitoring well, for a total of three (3) permanent groundwater monitoring wells. Permanent groundwater monitoring wells were selected in order to: 1) provide accurate evaluation of groundwater flow direction at the Site; and, 2) to provide a means of ongoing monitoring at the Site, which is anticipated. Task 1: Ground Penetrating Radar Survey Utility Mark-out and Ground Penetrating Radar Survey North Carolina 811 (a utility protection service) was contacted to identify public underground utilities at the Site prior to initiation of the subsurface investigation. In addition, a ground-penetrating radar (GPR) survey was performed at the Site prior to drilling activities to protect unidentified subsurface utility lines and other unforeseen obstructions. Unidentified subsurface objects were not observed in the immediate vicinity of the well locations during the GPR survey. The location of each of the borings is summarized in the table below and presented in Figure 2. A Geoprobe 7720DT mobile drill rig was used to advance each soil boring. During advancement of the bore hole, soil cuttings were screened for volatile organic compounds (VOCs) at one-foot intervals using a calibrated MiniRae 2000 photo-ionization detector (PID). Detailed soil boring logs are included in Attachment B. The following table provides a brief description of each boring advanced at the Site. PID detections were not identified above background levels (>1.2 ppm) in soils screened throughout the boring. One soil sample was collected from the depth interval of 14 to 16 feet below ground surface (bgs) and immediately above the water table. Groundwater was first encountered at a depth of 17.0 feet bgs. The boring was then advanced to a total depth of 30 ft bgs and converted to a permanent groundwater monitoring well (MW-6). SB-7 was advanced along the northeastern Site boundary and north of the Site building. The boring was advanced to a total depth of 29.1 feet bgs before encountering refusal on presumed weathered bedrock. PID detections above background levels were not identified in soils screened throughout the boring. One soil sample was collected from the depth interval of 12 to 14 feet bgs. Groundwater was encountered within this boring at a depth of 15.1 feet bgs. The boring was then advanced to a total depth of 29.1 feet bgs and converted to a permanent groundwater monitoring well (MW-7). SB-8 was advanced along the northwest Site boundary. PID detections above background levels were not identified in soils screened throughout the boring. One (1) soil sample was collected at the depth interval of 13-15 feet bgs, where HRP personnel observed a green-colored fill-dirt. A second soil sample was collected at the depth interval of 23-25 feet bgs, where native soils were believed to have been encountered. Groundwater was first encountered at a depth of 10.5 feet bgs. The boring was then advanced to a total depth of 34 feet bgs and converted to a permanent groundwater monitoring well (MW- 8). Soil Investigation A total of four (4) soil samples were collected from the three (3) borings advanced at the Site. Each soil sample was collected at the depth interval above the soil/groundwater interface, or where PID readings were observed above background levels. 527 Atando Avenue November 1, 2013 Page 3 of 6 HRP Associates, Inc. Each monitoring well was constructed of a 2-inch PVC riser with a ten (10) foot 0.010-slot screen interval. A filter-pack, composed of silica sand, was extended two (2) feet above the top of the screened interval. The filter-pack was followed by two (2) feet of bentonite chips and then tremie grouted to the ground surface. Each monitoring well was completed at the surface with a flush-mounted well cover and was set in a 2’ x 2’ concrete pad. Upon well completion and prior to groundwater sample collection, each monitoring well was developed using a decontaminated submersible pump and dedicated tubing. Monitoring well development was considered complete when turbidity was reduced and water quality parameters (i.e., turbidity, temperature, conductivity and pH) stabilized. Well development logs are included in Attachment C. Prior to collecting a groundwater sample, each monitoring well was purged using a decontaminated, disposable bailer. Each well was evacuated by removing a minimum of three (3) well volumes, or until water quality parameters stabilized. HRP personnel monitored water groundwater pH, temperature, turbidity, and conductivity at discrete volumes during well purging and immediately prior to sample collection using portable field instruments. These meters were calibrated daily before use per the manufacturer’s instructions and in accordance with HRP’s NC DENR laboratory certification requirements. Water quality data was collected in accordance with the EPA Region IV Science and Ecosystem Support Division’s (SESD) operating procedures. Groundwater sampling Field Data Sheets are presented in Attachment D. Task 3: Certified Land Survey Following the installation of the three (3) new permanent groundwater monitoring wells, a NC DENR accredited land surveyor performed a certified land survey measured from the top of the casings of each monitoring well at the Site (total 8 wells). During the survey MW-1 (located within the building) was discovered to have been abandoned and covered. Groundwater Flow Direction Depth to water was recorded within each well in order to estimate groundwater flow direction at the Site. Groundwater flow direction was calculated by contouring groundwater elevation data obtained via a certified land survey. Flow direction is estimated to be perpendicular to the elevation contours, or equipotential lines. On October 16, 2013 and prior to well purging, depth to static water level was measured in each viable Site monitoring well using a decontaminated air/water interface probe and recorded on a Water Level Log (Attachment D). Measurements were made to the nearest 0.1-foot from the surveyed top of the casing. Site groundwater generally flows south-southeast towards Little Sugar Creek. Hydraulic Gradient The hydraulic gradient was estimated using groundwater elevations measured at seven (7) on-site wells. The gradient magnitude was calculated using the equation: i =Δh/ΔL Where Δh is the change in head measured along the direction of groundwater flow, and ΔL is the straight-line distance between the measurement locations. A copy of the certified land survey is included as Attachment E. The calculated hydraulic conductivities and estimated linear velocities are included in Table 2. Task 4: Sample Analysis Soil Samples Four (4) soil samples were collected from the soil borings advanced at the Site on October 10, 2013. As previously noted, soil samples were collected from SB-6, SB-7, and SB-8. All soil samples were submitted for analysis under chain-of-custody to a certified North Carolina laboratory (Accutest Laboratories) and analyzed for the following constituents:  Volatile Organic Compounds (VOCs) – EPA Method 8260B In summary, no constituents were detected in soils above the applicable North Carolina Industrial Preliminary Soil Remediation Goals (IPSRG). Acetone was detected in the soil sample collected from SB-6; however, at a concentration significantly below the NC IPSRG. Analytical results of the soil assessment are summarized in Table 3. Soil concentration results were compared to the North Carolina Department of Environmental and Natural Resources (NCDENR) Industrial Preliminary Soil Remediation Goals (IPSRG). Laboratory reports, quality assurance/quality control (QA/QC) information, and chain-of-custody records are included as Attachment A. Groundwater Samples Groundwater samples were collected from the three (3) newly installed monitoring wells. Groundwater samples were submitted for analysis under chain-of-custody to a North Carolina certified laboratory (Accutest Laboratories). Groundwater samples collected from the monitoring wells were analyzed for the following constituents:  Volatile Organic Compounds (VOCs) – EPA Method 8260B Groundwater sample results were compared to the North Carolina groundwater standard established by the Division of Water Quality’s Classifications and Standards Unit. Regulations pertaining to groundwater quality standards are located in Title 15A of the North Carolina Administrative Code, Subchapter 2L, Sections .202 (15 NCAC 2L.0202). 527 Atando Avenue November 1, 2013 Page 5 of 6 HRP Associates, Inc. Creating the Right Solutions Together H:\G\Gould Family Properties II\GOU7000.GW - Atando Ave MW Install\WP\GOU4000.GW- Gould Atando Facility P2.docx In summary, the constituents detected above the applicable 15A NCAC 2L.0202 Groundwater Standards included:  Methyl chloride in MW-6  Methylene Chloride in MW-8  Tetrachloroethylene in MW-7 and MW-8  Trichloroethylene in MW-8 The constituents detected above reporting limits, but below the applicable 15A NCAC 2L.0202 Groundwater Standards included:  Chloroform in MW-6 and MW-7  Cis-1,2-dichloroethylene in MW-8 Groundwater analytical results are summarized in Table 4 and depicted on Figure 2. Laboratory reports, quality assurance/quality control (QA/QC) information, and chain-of-custody records are included as Attachment A. Investigative Derived Waste (IDW) disposal All soil cuttings and purge water generated during this assessment are temporarily staged on-site in six (6) 55-gallon steel drums. These drums were positioned to the west of the equipment shed, along the southwestern boundary of the paved portion of the Site on October 10, 2013. A label was placed on each drum noting its content, origin of materials (i.e. MW-6), and HRP’s contact information. Conclusions A Limited Phase II Environmental Site Assessment was performed on October 10, 2013 to further assess the extent of chlorinated solvents in groundwater at the Site (MW-5, F&R report dated June 2013) and to evaluate whether the source of contamination exists on-site, or is a result of an unidentified off-site source. Three (3) soil borings were advanced at the Site, which were subsequently converted to permanent groundwater monitoring wells. Four (4) soil samples and three (3) groundwater samples were collected and analyzed to evaluate soil and groundwater quality. Tetrachloroethylene was detected above the applicable regulatory standard in the groundwater samples collected within MW-7 and MW-8. Of note, both MW-7 and MW-8 are located hydraulically up-gradient of the Site building. Trichloroethylene was detected in MW-8 above the applicable regulatory screening standards. Cis-1,2-dichloroethylene, a breakdown product of trichloroethylene, was also detected in well MW-8. Methylene chloride was also detected above the applicable regulatory standard in the groundwater sample collected from well MW-8. Methyl chloride, a breakdown product of methylene chloride, was detected in well MW-6 above the applicable regulatory standards. Chloroform, a parent product of methylene chloride was detected in wells MW-6 and MW-7. The concentrations of tetrachloroethylene and trichloroethylene detected in MW-6, MW-7, and MW-8 were significantly less than what was observed in well MW-5 on May 31, 2013. The calculated area groundwater flow is to the south-southeast. Due to the fact that MW-5, MW-7, and MW-8 are positioned hydraulically upgradient of the Subject Site, and the fact that chlorinated constituents were not previously detected in the down-gradient wells MW-2, MW-4, DW-1, or MW-3, the presence of chlorinated solvents in Site groundwater is likely a result of an off-site, unidentified source. 527 Atando Avenue November 1, 2013 Page 6 of 6 HRP Associates, Inc.
