Dear Ms. Witt:

The North Carolina Division of Waste Management, Solid Waste Section (Section) has reviewed
the Permit Waiver Request (DIN 27362) submitted on February 5, 2017 for the Groundwater
Assessment Work Plan at the Mayo Steam Electric Plant. The request concerns Permit
Condition Attachment 3, Part 1, #11 which requires a licensed geologist be present to supervise
the installation of groundwater monitoring wells. Duke requests a waiver from this
requirement. In lieu of the requirement, Duke will have a Professional Geologist (P.G.) on call
during drilling activities. A field technician, under the supervision of a P.G., will be present
throughout the drilling activities.

In addition, the request concerns Permit Condition Attachment 3, Part IV, #21 which requires all
well construction records and boring logs to be submitted to the Section Hydrogeologist within
30 days of well installation. Duke requests a waiver from this requirement and proposes the
well construction records and boring logs be submitted with the final assessment report.

The Section grants the waiver request.
