The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Took soil samples, three boxes (two for existing fields, one for possible new field in front). Received check for
NCDA. Did not review site logs at this time.
2. The required signs are posted and the access road is adequate.
3. The cover crop has been planted. Land application shall occur this winter only if the cover crop is adequately
established.
4. Noted some scattered matted areas (heavy application) and thin/sparse crop areas. Ensure waste is being land
applied in a wide thin spray path. Some repair work may be needed this spring on the thin patches.
5. It appears that the approved setbacks are being met. No sign of land application outside the permitted
boundaries. Field markers appear accurate.
6. No recent application. No odors noted.
7. Discussed possible new area at the front of the site with Chester Cobb (site permitter) and reviewed setbacks.
Due to less neighboring vegetation was possible to use the range finder better than last visit. A 500’ setback is
required from the closest house in the neighboring subdivision (south side) which significantly impacts this new
area. A narrow rectangular area on the north side of field 1 may meet the required setbacks. If you wish to
pursue a permit modification to include this possible area, please contact Chester Cobb.
