or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle (for initial permit) tag#YA131845, 1M1AA13Y2TW059353, 3500 gallon capacity. Tank
appears structurally sound and securely attached.
2. Outlet covers were on and no drips or leaks were noted.
3. The lettering is correct on both sides of this vehicle.
4. Gave Mr. Smale an example truck log for the required information to be recorded once the firm permit is
issued.
5. I notified the Raleigh central office that the truck was in compliance and that Mr. Smale wanted to be informed
when the firm permit was issued.
