STATUS OF PAST NOTED VIOLATIONS:
The observed violation for field markers noted on the audits of 4/13/2016 and 11/17/2016 has not been corrected
although Mr. Woodall was informed of the issue during the site visit on 11/17/2016.
OBSERVED VIOLATIONS:
1. .0838(a)(10) Disposal area boundaries shall be clearly marked on the ground while a site or any part of a site is
in use. There are missing markers along the dividing line between fields 6 & 7. There are also missing markers at
the lower side of field 6. Repost the needed markers immediately. Since this violation was cited on
previous audits and was also discussed with Mr. Woodall on 11/17/2016 with no corrective actions taken as
of this date, further enforcement action may be needed. Please inform this office when the needed
corrective actions have been taken.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The required signs are posted at the gate entrance. The access road is adequate.
2. It appears that the approved setbacks are being met. No sign of land application outside the permitted
boundaries.
3. No sign of recent application. No odors noted.
4. This office has not received a revised field crop list (with signature & date) for the nutrient management
plan in regards to field 8 having a cropstand of fescue. This was discussed with Mr. Woodall on
11/17/2016. Please inform this office if the current field crop list (for the nutrient management plan) is to
be followed (field 8/small grains or millet).
5. Also noted that field 9 (which is supposed to be fescue) has been disced since the last visit on 11/17/2017.
