.0838(a)(10) & Permit Condition 18 Vegetation has been removed from around field markers. Discussed site map &
boundaries with Mr. Brock. The dividing line between the two permitted fields is present.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Took soil samples, two boxes. Received check for NCDA. Did not review site logs at this time.
2. The required signs are posted. The access road will need maintenance in the future. While on site a man who
identified himself as the landowner asked if it was possible to lock the entrance to the site to control trespassers.
I gave him my business card and asked for a key to be sent to me if this occurs to allow for inspection
access.
3. The winter cover crop has been planted. Land application shall occur this winter only if the cover crop
becomes adequately established.
4. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are being
met.
5. Noted hay bales sitting at the field perimeter which need removed when possible. Also noted that the
neighboring woodline/vegetation is encroaching onto the permitted fields & will need cut back to maintain the
permitted disposal area and prevent spraying onto this adjacent vegetation.
6. Noted some areas of heavy application/ matting – ensure adequate truck speed, pressure & spreader attachment
are used to allow for a wide thin spray path that does not damage the cropstand.
7. Noted areas of weeds & vegetation other than the approved crop. Please continue efforts to maintain the
Bermuda and control weeds.
8. No sign of recent application and no odors noted.
