Robinson, Haynes STATUS OF PERMIT: Active Issued: 10/4/2011 Expires: October 4, 2016 PURPOSE OF SITE VISIT: Routine Inspection Soil Sampling STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. All photographs were taken by Troy Harrison on November 3, 2015. 2. The Septage Land Application Site consists of Field 1 (2.5 acres), Field 2 (13.0 acres), Field 3 (7.0 acres) and Field 4 (8.0 acre3s) for a total of 30.5 acres. Field 4 is inactive at this time until trees are removed and approval by the Solid Waste section is given for use. Total usable acreage is 22.5 acres. 3. The site is permitted for land application of domestic septage (including portable toilet waste) and grease septage. 4. The site is located on at the intersection of Highway 18 and Glade Valley Road and has accessible all-weather roads to the septage land application field covered under the permit. 5. Appropriate signage was displayed at the site. 6. Disposal area boundaries were well delineated with permanent markers. 7. All septage discharges appear to have been made within the disposal area boundary. No recent septage application were observed. 8. No evidence of standing liquid or pooled waste was observed. The waste appeared to be evenly distributed throughout the field. 9. No objectionable odors were observed. 10. The nutrient management plan appears to have been followed on all fields with a good crop stand observed. 11. A residence was observed being constructed by the property owner in Field 2. It was observed that land applications are no longer being made within 500 feet of the construction site. During the inspection, a new 500 foot buffer was measured from the house. A 500 foot setback must be maintained as soon as a well is drilled or the house occupied. This field will be measured using a GPS in the coming months regards to the reduced acreage to be reflected in the 2016 permit renewal. Looking south at new home construction in Field 2 12.
