Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1. All photographs were taken by Troy Harrison on October 12, 2016. 2. The Septage Land Application Site consists of Field 1 (2.5 acres), Field 2 (13.0 acres), Field 3 (7.0 acres) and Field 4 (8.0 acre3s) for a total of 30.5 acres. Field 4 is inactive at this time until trees are removed and approval by the Solid Waste Section is given for use. Total usable acreage is 22.5 acres. 3. The site is permitted for land application of domestic septage (including portable toilet waste) and grease septage. 4. The site is located on at the intersection of Highway 18 and Glade Valley Road and has accessible all-weather roads to the septage land application fields covered under the permit. 5. Appropriate signage was displayed at the site. 6. Disposal area boundaries were delineated with permanent markers, however, some of the markers were knocked down or missing. These will need to be replaced. The marker at the northeast corner of Field 1 may not be at the corresponding permitted location. It is the responsibility of Haynes to ensure that the boundary markers are in their correct location. 7. All septage discharges appear to have been made within the disposal area boundary. 8. No evidence of standing liquid or pooled waste was observed. The waste appeared to be evenly distributed throughout the fields. Field 1 and a portion of Field will needs reseeding. As noted in the previous inspection, a residence was observed being constructed by the property owner in Field 2. It was observed in the previous and current inspections that land applications are no longer being made within 500 feet of the construction site or water supply well. The field will need to be re- measured for the current permitting cycle.
