The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. All photographs were taken by Troy Harrison on October 4, 2016.

2. The Septage Land Application Site (site) consists of Field 1 (3.50 acres), Field 2 (3.19 acres) and Field 3 (5.02 acres) for a total of 11.71 usable acres.

3. The site is located at 3639 Beasley School Road, Snow Creek, NC. 4. The entrance road was in good repair.
5. Field disposal area boundaries were well delineated with permanent markers corresponding to the site map. A
few markers were obscured by woody vegetation which will need to be removed.
6. All septage discharges appear to have been made within the disposal area boundary.

7. Fields 2 and 3 appeared to be in good condition with no evidence of standing liquid or pooled waste.
8. Field 1 appears to be improving conditions due to previous overloading and ruts; however, bermuda
grass and weeds appear to be increasing. The approved Nutrient Management Plan contained in your permit has fescue as the cover crop for all fields and requires reseeding if the fescue crop falls under 85% coverage. You may contact Connie Wylie at [e-mail] or 910-433-3352 for additional
