The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The permitted Septage Detention and Treatment facility consist of one 10,000 gallon metal tank (tank) for the storage of domestic septage from recreational vehicles. The waste from any recreational vehicle that is being used
as a residence is prohibited from being stored in this tank.
2. Appropriate signage was in place along access roads.

3. Vegetative growth was effectively controlled in the areas around the tank.
4. No leaks or spills were observed. It appears that the tank has not been used in some time.
5. The tank appeared to be structurally sound.

6. Vectors were not present at the time of inspection nor were objectionable odors observed.
7. Approved setbacks and buffers for the tank appear to have been met. 8. A new GPS reading was taken using a Trimble unit.

9. Tyana Miller is no longer with the company. The current permit is issued to Lake Myers RV Resort and Tyana Miller. A permit revision will need to be issued to reflect that change. A permit application, LLC
Landowner Authorization Form and Cease Operation form were given to facility staff. If Lake Myers RV Resort wishes to continue using the tank, please submit the application and landowner authorization form by December 9, 2016. If the resort wishes to discontinue using the tank, then the tank will need to be
emptied by a certified septage management firm and the cease operation from completed by December 9, 2016.
