The company and land were sold to Enviro-Choice in September 2016. As of this date this office has not received an application submittal to reissue this SLAS permit to reflect these changes. Within 30 days submit the required items for the permit applications submittal. SEE BACK The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Took soil samples, five boxes. Received $125 check for NCDA. Reviewed SLAS log, it appears that the required information is being maintained. 2. It appears that the winter cover crop has been planted. Land application shall occur this winter only if the cover crop becomes adequately established. 3. The field markers appear accurate. It appears that the approved setbacks are being met/not measured at this time 4. No sign of land application outside the permitted boundaries. 5. No sign of recent or heavy application. No odors noted. 6. The required signs are posted and the access road is adequate. 7. The truck cleanout area appears properly contained. 8. Hay bales located along field perimeters need to be moved when possible. 9. Discharge events have been decreased to give cropstand a rest. Noted scattered thin patches during this visit. Please implement repair work & weed control in the spring as needed to adequately maintain the Bermuda.
