On behalf of Waste Industries of High Point, LLC (Waste Industries), Golder Associates NC, Inc. (Golder) is submitting the enclosed letter report summarizing the installation groundwater assessment wells
AMW-1 and AMW-2 at the active Waste Industries of High Point Construction and Demolition (C&D) Landfill in Guilford County, North Carolina (NC). The assessment wells were installed in accordance with
the Assessment Monitoring Notification and Work Plan Addendum (AMNWPA), dated October 21, 2016, which was approved by the NC Department of Environmental Quality (DEQ) in a letter dated October 31,
2016. Landfill gas monitoring well LFG-1 was not installed, as proposed in the AMNWPA, due to shallow groundwater (approximately 3.5 feet below ground surface [bgs]). The NC DEQ was notified by
telephone the day of the well installations of this change to the AMNWPA.
On December 5, 2016, a Golder licensed geologist provided oversight for the installation of assessment
wells AMW-1 and AMW-2 in accordance with Title 15A of the North Carolina Administrative Code (NCAC), Subchapter 2C .0100, the approved AMNWPA, and Draft North Carolina Water Quality
Monitoring Guidance Document for Solid Waste Facilities; Solid Waste Section, Division of Solid Waste
Management; Department of Environment, Health and Natural Resources (March 1995). The well was
installed by South Atlantic Environmental Drilling and Construction Company, Inc. (SAEDACCO) of Fort Mill, South Carolina using a Geoprobe 3230DT drilling rig utilizing 4.25-inch inner diameter (ID) hollow
stem augers (HSAs) and a 4-inch downhole hammer.
The borings were advanced with HSAs to the approximate saprolite / bedrock interface at 9 feet bgs at
AMW-1, and at 3.5 feet bgs at AMW-2. Afterwards, the borings were advanced utilizing air drilling methods with a downhole hammer to the target depth of 15 feet bgs based on static groundwater levels.
The assessment wells were constructed with 2-inch schedule 40 polyvinyl-chloride (PVC) screen and riser pipe. The 0.010-inch slotted screens were both installed from 5 to 15 feet bgs. The filter pack
consisted of Driller Service Inc. (DSI) #2 sand from 4 to 15 feet bgs. A filter pack seal was installed from 2 to 4 feet bgs and consisted of 3/8-inch bentonite chips. The remaining annular spaces were filled with
Type I Portland cement. The surface completions consisted of locking 4-inch aluminum protective casings and a 3-foot by 3-foot by 0.5-foot concrete apron. A summary of well construction information is
provided on Table 1. The boring / well construction log and Form GW-1 Rev. 8/13 for each well are provided as Attachment A.
The wells were developed on December 16, 2016, utilizing a Waterra inertial pump. AMW-1 was developed for 4 hours and approximately 57 gallons of water was removed. AMW-2 was developed for
3.25 hours and approximately 52 gallons of water were removed. The well development forms are provided as Attachment B. Though turbidity values remained high during development, during sampling
of the wells on January 12, 2017, turbidity values for each well were under 10 Nephelometric Turbidity Units. The wells were surveyed by Regional Land Surveyors, Inc. of Greensboro, NC on December 16,
