During this visit I did not see any land application outside the permitted boundaries.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Took soil samples at this time, two boxes. Received $50 check by mail for NCDA.
2. No sign of heavy or recent application. No odors noted.
3. The field markers appear accurate – however excess vegetation needs to be removed from markers at the back
of field 1.
4. It appears that the approved setbacks are being met. No sign of application outside the permitted boundaries.
5. The cover crop shall be adequately established for land application to continue thru the winter months.
6. Remove hay bales from the perimeter of field 1.
7. The required signs are posted & the access road is adequate.
8. I requested a copy of the land application site logs to review. However, I received a copy of the truck pumping
log which is enclosed. Please submit a copy of the land application site logs (covering 2016) so that I may
review the required information. An example log is enclosed.
