Central Carolina Tire Processing
Permit No. 4308-TIRETP-, Harnett County, Document Id No. 27170

Dear Mr. Frye:

Please find the enclosed reissue of the permit approval to operate for the Central Carolina Tire Processing Facility,
Permit No. 4308-TIRETP-. After issuing the permit on December 16, we noticed a few errors, primarily concerning
financial assurance, that might lead to confusion. The permit has the same document identification number (DIN),
but a new issue date of December 19, 2016. Please replace the copy I sent you earlier with this one.

These are the changes made to the permit conditions.
1. In the permitting history table under Attachment 1, Part II. The first permit listed should have been Permit
43-04, the permit number for the monofill, not 43-03.
2. Condition 11 in Attachment 3, Part I was modified to clarify that the financial assurance update is due on the
anniversary date of the financial mechanism.
3. Condition 13 in Attachment 3, Part II stated that changes in financial assurance mechanisms must be
submitted at least 120 days prior to requested changes. This had been in previous permits. The 120-day
advance notice is not a requirement and was therefore removed.
4. Condition 13 (c) stated that the financial assurance mechanism must be reviewed and adjusted annually by
August 15. This had been in previous permits and may have been based on a specific situation in the past.
As stated above, the adjustment is due on the anniversary date of the financial mechanism. Therefore, this
condition was removed.
