OBSERVED VIOLATIONS:
1. .0838(a)(18) “Approved nutrient management plans shall be followed” & Permit Condition 2 “This site shall be
operated and maintained in accordance with the nutrient management plan submitted by Dennis Hoyle and
approved by the Division. A review of the submitted land application site logs revealed that the monthly
application rates (per the approved nutrient management plan) were exceeded on the following months (all low
monthly application rates).
January 2013, Field 1 – 34,500 gallons applied (only 31,650 gallons allowed for 6.33 acres x 5,000 gallons).
June & July for both 2013 & 2014, Field 2 – 15,000/19,500/17,500/ & 20,000 gallons applied (only 11,400
gallons allowed for 2.28 acres x 5,000 gallons).
Please review the nutrient management plan and implement measures to monitor the application amounts each
month, particularly on a “low” month, to prevent exceeding the approved application rate. Please contact this
office if you have any questions.
