NAME OF FACILITY: Hanes Mill Road Landfill DATE AND TIME OF FIRE: 10/28/2016 HOW WAS THE FIRE REPORTED AND BY WHOM: @ 1100 hrs to 1430 hrs Solid Waste Engineer, Ed Gibson, called Susan Heim ofNCDEQ, by telephone on 10/28/2016, 12 noon. LIST ACTIONS TAKEN: PERMIT # 34-02 Fire Department was called. Fire was contained within landfill cell #5 presently under construction, and occurred within the leachate collection system. The fire was extinguished by constructing cut-off trenches immediately adjacent to the two burning leachate pipes and placing soil atop the leachate collection pipes, thereby starving the fire of oxygen. WHAT WAS THE CAUSE OF THE FIRE: Present indications are that the liner subcontractor was heat welding the separator geotextile (that which is placed between the leachate collection stone and the operational cover) near the new, 12 inch leachate collection header located along the toe of the western slope. This 12 inch header pipe had already been tied into the existing leachate collection system. This heat welding caused the landfill gas to ignite. DESCRIBE AREA, TYPE, AND AMOUNT OF WASTE INVOLVED: Fire occurred in cell #5, a new cell that was under construction. Landfill gas was burning, not waste. WHAT COULD HA VE BEEN DONE TO PREVENT THIS FIRE: DESCRIBE PLAN OF ACTIONS TO PREVENT FUTURE INCIDENTS: Determine best course of welding around potential landfill gas areas. He stated the leachate pipe in the area of the fire would be replaced. No action required in addition to the plans indicated above.
