Dear Mr. Davis:

Please find the enclosed permit approval to operate for Stericycle Environmental Solutions, Permit No.
7907-TRANSFER-2016. The approved operation plan for the facility is also provided (DIN 25953). A
pre-operative meeting with the environmental senior specialist in accordance with the permit condition 2
in Attachment 3, Part VI of the permit is required prior to accepting waste at the facility.

In general, the facility permit does not reiterate permit conditions which are included in the Solid Waste
Management Rules, 15A NCAC 13B, nor does it restate specific conditions which are included within the
approved plans. Please review the conditions of the permit thoroughly.

Also included is a certified original of the permit. The certified original is provided for recordation with
the Rockingham County Register of Deeds. A copy of the certified original affixed with the Register’s
seal and the date, book, and page numbers of the recording must be returned within 30 calendar days of
issue to the Permitting Branch Supervisor as noted at the bottom of the certification statement.
