OBSERVED VIOLATIONS:
1. .0838(a)(10) Disposal area boundaries shall be clearly marked on the ground while a site or any portion of a site
is in use. There are missing markers along the dividing line across the hill top between fields 6 & 7. There are
also missing markers at the lower swale side of field 6. A marker is down at the front right corner of field 15.
Please repair/repost these field markers. This was requested on the last audit of 11/23/2015 and it appears
that no corrective actions were taken.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The cropstand appears adequate – please control weeds in the coming months.
2. Recent rains – did not drive over entire site. No recent or heavy applications. No odors noted.
3. No apparent sign of land application outside the permitted boundaries. It appears that the approved setbacks are
being met.
4. The required signs are posted at the gate entrance. The access road is adequate.
