OBSERVED VIOLATIONS:
1. Upon review of the submitted land application site logs for permit renewal it was noted that the time of each
discharge was not recorded as required in 40 CFR 503. Please begin recording the time of discharge along with
the date that septage is land applied to the permitted site.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The cropstand appears much improved on both permitted fields. Noted some scattered thin patches of fescue
which may need some spot reseeding. Please control weeds in the coming months to maintain the fescue
coverage.
2. No sign of heavy application or recent application. No odors noted.
3. No sign of land application outside the permitted boundaries. Field markers are adequate. It appears that the
approved setbacks are being met.
4. The required signs are posted and the access road is adequate.
5. Cattle on field 3 during this visit.
