STATUS OF PERMIT:
Active. Permit to Operate (PTO) Cell 4 issued April 13, 2011, expires November 28, 2016. No later than June 01, 2016.
Dare County must send a permit review request must be sent to DWM for cells 1-4 to renew their PTO. An additional
Amendment has been sent to DWM for an approval for a Phase 5. This has been approved for PTO and Permit to
Construct (PTC) on August 28, 2015 and that permit will expire on February 28, 2017 if substantial construction does
not begin within 18 months from the issuance date of this permit; the permittee must obtain a permit modification to
construction. Phase 5 construction has begun.

ADDITIONAL COMMENTS;
01 – This Facility is permitted to receive Construction and Demolition (C&D) waste, inert debris, land clearing debris,
asphalt.
02 – Cell #4 is currently being operated. Cell #5 has begun construction.
03 – This facility is secured by locked gate at the facility entrance.
04 – The operating hours are Monday – Friday 7:00am to 5:00pm and Saturday 7:00am to 12:00 pm. Holidays observed
are New Year’s Day, Thanksgiving Day and Christmas Day.
05 – Facility has proper signage with all required information displayed.
06 – Facility roads are of all-weather construction and overall in good condition.
07 – Facility monitoring wells observed and appear to be maintained.
09 – This C&D permitted facility has 4 certified operators: Clyde Gard, expires October 19, 2019. Glen C. Bailey, expires
October 19, 2019. Deborah A. Equils, expires June 21, 2019 and Ashley Walston, expires May 24, 2019.
10 – The working face is limited to a small area (¼ acre) with cover added daily. Screening of the loads dumped are
being done. Any non C&D waste pulled out of the C&D working face is removed and taken to 28-05T-Transfer Station
on-site and White Goods and Metal pulled out are taken to the White Goods and Scrap Metal area. Cover material
stockpiled close to working cell.
11 – Waste edge markers in place. Some have been broken and need to be put back up.
12 – Overall, the capped cells of the Landfill appear to be maintained well.
13 – A Comprehensive Facility Compliance Inspection was done, all records and landfill footprint was reviewed.
14 – Digital Pictures taken during this Comprehensive Facility Compliance Inspection.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
