The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. All photographs were taken by Troy Harrison on March 10, 2016.
2. The Septage Land Application Site consists of Field 1 (13.8 acres), Field 2 (18.6 acres) and Field 3 (12.6
acres).
3. The site is permitted for land application of domestic septage and grease trap pumpings.
4. The site had accessible all-weather roads to the septage land application field covered under the permit.
5. Appropriate signage was displayed at the entrance to the fields.
6. All fields had disposal area boundaries that were delineated with permanent markers which corresponded to
the site map with the following exceptions:
a. The boundary between Fields 2 and 3 had small flags as boundary markers which may not be adequate
when the fescue is taller, and
b. A field marker was missing at the top of one of the swales on Field 2 (approximately midway up from the
pond).
7. The boundary between the southern boundary of Field 2 and a pond was approximately 100’, however, the
current map has noted 200’. The current file does not clearly state whether it should be 100’ or 200’. 15A
NCAC 13B .0837(d)(5)(D) sets a distance for this pond to 200’; however, 15A NCAC 13B .0837(d)(15) allows
for the setback to be reduced by 50% when septage is pretreated to accomplish pathogen reduction and when
the land within the setback area is in permanent, established grass with at least 95% cover. Mr. Lentz left a
message with Connie stating that Michael Scott, former Composting & Land Application Branch Head, had
reduced the setback based on 15A NCAC 13B .0837(d)(15). This appears to correspond with the aerial
photograph of Field 2 showing field boundaries.
8. All septage discharges appear to have been made within the disposal area boundary. Fields 1 and 2 appeared to
be well maintained with a good crop of fescue. Field 3 appeared to be having some problems with
weeds/vegetation other than fescue. Recent reseeding was observed in this area.

9. No evidence of standing liquid or pooled waste was observed.
10. No objectionable odors were observed.
11. The nutrient management plan appears to have been followed.
