The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1. All photographs were taken by Troy Harrison on March 10 2016.
2. This facility is currently permitted to operate two 10,000 gallon tanks and three 30,000 gallon tanks for the
storage of domestic septage and grease trap pumpings. One 10,000 gallon tank has been removed and the other
10,000 gallon tank is for water only.
3. A screening box (box) was observed just under half full. Part of the top of the box was covered and inside of
one corner was a six inch PVC pipe. There is a concern about vectors and how long the waste is stored in the
box before disposal. According to Mr. Lentz, a bar screen has been ordered to place in the box for better
collection of bigger trash items. A letter describing how the box is used has been received by the Solid Waste
Section. A dumpster was observed nearby with no sign of septage waste just general solid waste.
4. Current Set-back/buffer requirements appear to have been met.
5. Appropriate signage (No Trespassing signs) is posted for the site.
6. All weather roads are maintained for access to the site.
7. The tanks appeared to be structurally sound.
8. The area around the detention and treatment area was found to be free of spills and debris.
9. Odors observed in the area of the detention and treatment area but not off property boundary.
