ADDITIONAL COMMENTS
1. All Photographs were taken by Troy Harrison on March 22, 2016.
2. The Septage Land Application Site consists of two fields totaling 6.5 acres with Field 1 (3.1 acres) and Field 2
(3.4 acres).
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

3. The site is permitted for land application of domestic septage and grease septage.
4. The site is located on Dogwood Vista Road and has accessible all-weather roads to the septage land application
field covered under the permit.
5. Appropriate signage was displayed at the site
6. Disposal area boundaries were delineated with permanent markers.
7. All septage discharges appear to have been made within the disposal area boundary.
8. The waste appeared to be evenly distributed throughout the fields with the exception of a small area of Field 2
which had waste from a recent heavy application. As a reminder, septage shall be applied to the surface of the
land from a moving vehicle in such a manner as to have no standing liquid or soil disturbance resulting from the
waste flow after the discharge is complete.
9. The approved Nutrient Management Plan appears to have been followed.
10. No objectionable odors were observed.
