The lettering has been corrected on both sides of the vehicle – the firm name & town name are now at the required
height. Four additional bolts have been installed to secure the tank to the truck bed.

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The vehicle (tag#EA4688, pickup truck with 100 gallon tank in back) is now in compliance.
2. Please contact Michelle Sclafani in regards to required paperwork for the firm permit.
3. No septage jobs shall be performed until the firm permit is issued.
