The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected new vehicle - tag#YA130493, 1FV6HFAA5TL657963, 2500 gallon capacity. The lettering is correct
on both sides of this vehicle. The tank appears structurally sound and is securely attached. Outlet covers were
on and no drips/leaks were noted. The Raleigh office has received the additional $250 fee for this second
vehicle. This vehicle may be used for septage pump jobs.
2. Inspected older vehicle – tag#YA110424, VIN number is correct per the recent application. The tank appears
structurally sound and is securely attached. The new phone number is now displayed on this vehicle. The
lettering is correct on both sides of this vehicle. Outlet covers were on and no drips/leaks were noted.
3. Reviewed truck logs – it appears that the required information is being maintained, but please ensure that the
full physical address is listed for each pump job.
4. Discussed steps to obtain a land application site permit. Information enclosed.
5. Mr. Wilkins attended the January training class in Raleigh.
