or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Tank was switched to another truck. Tag#BM-8169, VIN 3FDNF65634YMA19411. Tank is structurally sound
and securely attached.
2. Lettering is displayed all on the tank sides - no change in lettering and is adequate.
3. Outlet covers were on. No drips or leaks noted.
4. Reviewed truck & site logs, it appears that the required information is being maintained for both records.
5. Mr. Dickerson attended the January training class in Hickory.
