OBSERVED VIOLATIONS:
1. .0844(b) The decals or lettering shall include the firm name, town name, phone number and permit number and
be no less than three inches in height. Lower case letters in the firm name & town name were less than three
inches. Please correct the lettering on both sides of the truck. The phone number & permit number were correct.
2. .0844(a)(1) All tanks shall be constructed of metal and permanently attached to the truck bed. The 100 gallon
tank in the pickup truck bed is currently only attached with two bolts. Please install additional bolts.

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected new vehicle in regards to issuance of new firm permit. Correct tag# & VIN per the application.
2. Tank appears structurally sound & outlet covers were on.
3. Follow up inspection will be performed in regards to lettering & installation violations.
4. No septage pump jobs shall be performed until the firm permit is issued.
