Issued May 27, 2015 – Expires, February 26, 2020 PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS; 1 - The PTO, Ops Plan and Contingency plan were on site and reviewed. 2 - The service area is Pasquotank and Camden Counties. 3 - The operating hours are M - F 8:00AM to 4:30PM and 8:00AM to 12:00PM Saturday. 4 - Waste must be transported to WM-Atlantic Waste Disposal Landfill in Waverly Va. or transported to East Carolina Regional Landfill (permit# 08-03) in Aulander, Bertie Co. 5 - Waste screening records were acceptable. It appears an average of 2 loads per week (at a minimum) are screened. 6 - The tipping floor and building were as clean as possible. 7 - Signage is in compliance with permit and rule conditions. 8 - The access roads allow for all weather conditions. 9 - Scrap metals are removed from the tipping floor and placed in a roll off container. 10 – Facility has tank for oil recovery if found to be in any load. 11 - Site does a good job containing blown waste. 12 – Leachate tanks are being pumped out on an as needed basis. Tanks inspected daily. 13 - Filled trailers are taken to landfill as soon as full. Site stores empty trailers on concrete pad and sloped towards the front of trailers to prevent leakage. 14 – The current Certified Transfer Station Specialist are listed below with expiration dates included. Janice V.
