Dear Ms. Werner,

Per the Section’s approval on July 27, 2016 titled Approval of Replacement monitoring Wells MW-5 and
MW-7, MW-5 and MW-7 have been replaced and the original wells abandoned. Attached, in accordance
with Attachment 3, Part IV, Item 21 of Permit No. 6004, Duke Energy is proving the well completion
records and boring logs for replacement wells MW-5R and MW-7R.

Duke Energy is also providing well abandonment records for two (2) non-water supply monitoring well at
Duke Energy’s McGuire Landfill No.2 in Huntersville, North Carolina abandoned on October 11, 2016.

Per 15A NCAC 02C .0114 (b)(2), these records were submitted within 30 days of well completion or
abandonment.

Records are provided for the following non-water supply monitoring wells:

MW-5
MW-7
MW-5R
MW-7R
