The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Once a year on site meeting, received $25 check for NCDA for soil sample & discussed requirements for land
application site logs. Took one soil sample on the active field (#2) only. Field #1 is inactive.
2. Discussed that the millet crop on Field #1 is not being used for human consumption, so no 50’ buffer required
between Fields 1 & 2.
3. Weed control is needed in the future, noted scattered areas of vegetation other than fescue & weeds on field #2.
Please continue efforts to maintain/improve the fescue cropstand.
4. No sign of recent land application. No activity for a long time. No land application outside the permitted
boundaries or heavy application noted. No odors.
5. The field markers are present at the swale area, but there are missing markers at the back of field #2 separating
it with the neighboring field/crop. However no land application is occurring on field #2 at this time.
6. The required signs are posted at the entrance gate and at the permitted fields. Access road was adequate.
