STATUS OF PERMIT:
Active, the Permit to Operate was issued on August 6, 2012 and is set to expire December 19, 2016. No later than July
19, 2016 Martin Co. must submit to the Section documents for a permit amendment. This approves a modified facility
plan that increases the slopes to 3, 1adds one acre to the boundary and is for Fill Sequences 1.

None

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS

1) 1 - The Permit and Operations Plan were on site and available for review and were reviewed. The operator has
set up a drawer for SWS files.
2) The hours of operation are 8AM-4:30PM, MF and 8AM to 12PM Saturday.
3) The service area is Martin Co.
4) Scrap tires are being stored in closed trailers and are generally being managed well.
5) The certified operators are: Maurice Robinson, expires 10/16/18; Johnny Rascoe, expires 12/06/18; Ronald
Brownfield, expires 10/19/16.
6) CFC containing white goods are being separated from the non CFC containing white goods, stacked neatly and
numbered. FOSS Recycling is collecting the white goods and is providing records of CFC removal.
7) Waste Screening records are good and detailed.
8) The last GW monitoring event was in 11/16/15. The GW Monitoring Wells were locked as required.
9) The edge of waste markers are in place and acceptable. Need to replace a few.
10) Signage is according to the permit.
11) Electronics are being stored under a shelter.
12) A Methane Monitoring Plan is included in the new permit. Methane wells were monitored quarterly in 2014 &
2015. Last time monitored was 02/29/16.
13) Scrap metal & white goods area on concrete pad and looks to be managed well.
14) Working face of CDLF is small. Due to constant rainfall, the covering has been a challenge. Cover is not bad
but as soon as the weather allows, please add a bit more cover. Small amount of surface water at toe.
15) Closed MSWLF has signage and appears to be maintained well. Watch small tree growth on side slopes.
