with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:

01 - This Facility Compliance Inspection was un-announced.
02 - The site is being used by Al Collie with Collie’s Landscaping.
03 - The landfill is located on property owned by Sidney A. Collie Jr. and Leah, a 32.99 acre tract Parcel ID# 025622
recorded in Book 1815 Page 932 of the Nash Co. Register of Deeds.
04 - The two acre boundary of the landfill should be staked to keep the filled area within the requirements.
05 - Mr. Al Collie was called when I arrived to site.
06 - Land clearing and Inert Debris was onsite. Some metal and creosote timbers were mixed into the Inert Debris and
was being screened out of waste stream. Metal for recycle and creosote timbers needs to go to permitted landfill.
07 - Overall site appeared to be manage well. The East Side Slopes needs to be shaped up a bit to prevent erosion.
Facility has plenty of cover material at this time.
