PURPOSE OF SITE VISIT:

STATUS OF PAST NOTED VIOLATIONS:
1. 15A NCAC 13B .1626(7)(b): Not resolved, Corrective Measures in Progress. Side slopes in Phases 1-4 have
been reseeded and portions of the slopes have been matted.
2. 15A NCAC 13B .0203(d): Not resolved. Corrective Measures in Progress. Side slopes in Phases 1-4 have
been reseeded and portions of the slopes have been matted.
OBSERVED VIOLATIONS:

ADDITIONAL COMMENTS
1. All photos were taken by Lee Hill on November 1, 2016
2. The facility is currently filling in Phase 5 of the landfill.
3. Minimal windblown litter was observed and was being well managed at the time of inspection.
4. The working face was small, compact and appeared to be able to be covered with approved alternative daily
cover.
5. No unacceptable waste was observed on the working face at the time of inspection.
6. The addition of intermediate cover to Phases 1-4 has been completed. The phases have been seeded, mulched
and some vegetation has begun to emerge. The facility only received 0.4 inches of rain during the month of
October and due to the lack of rain the germination of groundcover has been slower than expected.
7. Portions of the slopes in Phases 1-4 have been matted to help prevent erosion.
8. Slope drains and berms have been installed in Phases 1-4.
9. The facility is in the process of upgrading Edge of Waste Markers in Phases 1-4.
10. Significant progress has been made to address the issues noted in the August 3, 2016 Notice of Violation. When
groundcover sufficient to restrain erosion is established in Phases 1-4 the outstanding issues can be resolved.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
