This Phase I ESA was developed and performed to meet all appropriate inquiries in generalconformance with the standards and practices set forth in 40 CFR Part 312. Keith C. Seramur - Senior Geologist (;(&87,9(6800$5< 6XEMHFW3URSHUW\'HVFULSWLRQ The subject property is known as Blowing Rock Hospital and is located at 364 & 366 Chestnut Drive, Blowing Rock, NC. The Property is located on the west side of Chestnut Drive, approximately 160 feet south of the intersection with Henkle Street. The location of the Property is shown on Figures 1, 2 and 3 in Appendix A. The Property consists of approximately 3.61 acres and contains one residential building and one large medical complex. The site is currently occupied by Blowing Rock Rehabilitation & Davant Extended Care Center. 'DWD*DSV A data gap is defined by ASTM Standard E 1527-13 as a failure of or inability to obtain information required by this practice despite good faith efforts by the environmental professional to gather such information. The following data gaps were identified during performance of the Phase I ESA: The ASTM method requires documentation of a property in approximately 5-year intervals back to its first developed use, or back to 1940, whichever is earlier. Property use could not be determined for the period of 1942 to 1959. Historical resources for this period were not reasonably attainable. 1 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 (QYLURQPHQWDO5HSRUW6XPPDU\ Seramur & Associates, PC has performed an Environmental Site Assessment, in conformance with the Scope of Work developed in cooperation with the client and the provisions of ASTM Practice E 1527-13. Any exceptions to, or deletions from, this practice are described in Sections 2.2, 3.4 and 3.5 of this report. The user of thisPhase I will need to decideif additional investigation inthe vicinity of the formerAST is warranted for agreater comfort level.  3LWV3RQGV$QG/DJRRQV X  6ROLG:DVWH'XPSLQJ/DQGILOOV X  6WDLQHG6RLO6WUHVVHG9HJHWDWLRQ X  :HOOV X 5HFRPPHQGDWLRQV This Phase I ESA has revealed evidence of recognized environmental conditions or significant environmental concerns in connection with the subject property. NC Department of Environmental Quality assigns responsibility for a release from a UST system to the owner/operator of the UST at the time that the release is discovered. Seramur & Associates recommends that the UST system be properly closed by removal prior to Brackett Flagship Properties taking possession of the property. 3 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 The source of heating and incinerator fuel for the medical clinic between 1954 and 1964 is not known. Did this facility use a heating oil UST? If there is an abandoned UST in the vicinity of the former medical clinic, it could be located with a geophysical survey. It is ultimately the responsibility of the user of this Phase I ESA to decide if further investigation is necessary to achieve the desired comfort level. ,1752'8&7,21 3XUSRVH The purpose of the Phase I Environmental Site Assessment (ESA) was to evaluate the current and historical conditions of the Subject Property in an effort to identify recognized environmental conditions in connection with the Subject Property. A recognized environmental condition is defined by ASTM as: Recognized Environmental Condition - The presence or likely presence of any hazardous substances or petroleum products in, on, or at a property due to release to the environment; under conditions indicative of a release to the environment; or conditions that pose a material threat of a future release to the environment. De minimis conditions are not recognized environmental conditions. The identification of recognized environmental conditions in connection with the subject property may impose an environmental liability on owners or operators of the site, reduce the value of the site, or restrict the use or marketability of the site, and therefore, further investigation may be warranted to evaluate the scope and extent of potential environmental liabilities. 6FRSHRI:RUN The Phase I ESA conducted at the Subject Property was in general accordance with ASTM Standard E 1527-13 and included the following: • Records review; • Interviews with personnel associated with the subject and adjoining properties; • A site visit; and • Evaluation of information and preparation of the report provided herein. Typically, a Phase I ESA does not include sampling or testing of air, soil, groundwater, surface water, or building materials. These activities would be carried out in a Phase II ESA, if required. For this Phase I ESA, no additions to the ASTM E 1527-13 standard were made. 6LJQLILFDQW$VVXPSWLRQV There is a possibility that even with the proper application of these methodologies there may exist on the Subject Property conditions that could not be identified within the scope of the assessment or which were not reasonably identifiable from the available information. Seramur & Associates, PC believes that the information 4 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 obtained from the record review and the interviews concerning the site is reliable. However, Seramur & Associates, PC cannot and does not warrant or guarantee that the information provided by these other sources is accurate or complete. The methodologies of this assessment are not intended to produce all inclusive or comprehensive results, but rather to provide Brackett Flagship Properties with information relating to the Subject Property. /LPLWDWLRQVDQG([FHSWLRQV Along with all of the limitations set forth in various sections of the ASTM E 1527-13 protocol, the accuracy and completeness of this report may be limited by the following: Access Limitations - The facility is a rehbilitation center and long term care facility for elderly residents. We toured and observed serval patients rooms and showers, but did not access every room in the facility out of respect to the residents. ASTM Standard includes observing all interior space. We do not believe that omitting observations inside patients private rooms has prevented us from identifying RECs associated with the property. It should be noted that this assessment did not include a review or audit of operational environmental compliance issues, or of any environmental management systems (EMS) that may exist on the property. Where required, the documents listed in Appendices C and D were used as reference material for the completion of the Phase I ESA. Some of the information presented in this report was provided through existing documents and interviews. Although attempts were made, whenever possible, to obtain a minimum of two confirmatory sources of information, Seramur & Associates, PC in certain instances has been required to assume that the information provided is accurate. The information and conclusions contained in this report are based upon work undertaken by trained professional and technical staff in accordance with generally accepted engineering and scientific practices current at the time the work was performed. The conclusions and recommendations presented represent the best judgment of Seramur & Associates, PC based on the data obtained from the work. Due to the nature of investigation and the limited data available, Seramur & Associates, PC cannot warrant against undiscovered environmental liabilities. Conclusions and recommendations presented in this report should not be construed as legal advice. Should additional information become available which differs significantly from our understanding of conditions presented in this report, we request that this information be brought to our attention so that we may reassess the conclusions provided herein. Instructions as to the location of the property, access, and an explanation of the property and facilities to be assessed were provided by Brackett Flagship Properties. 5HOLDQFH This report has been prepared for the sole benefit of the Brackett Flagship Properties. The report may not be relied upon by any other person or entity without the express written consent of Brackett Flagship Properties and Seramur & Associates, PC. 6,7('(6&5,37,21 /RFDWLRQDQG/HJDO'HVFULSWLRQ The subject property is known as Blowing Rock Hospital and is located at 364 & 366 Chestnut Drive, Blowing Rock, NC. Information obtained from the Watauga County Tax Administration shows the parcel number for the subject property as 280-79-6349-2000. $FWLYLW\8VH/LPLWDWLRQV Seramur & Associates, PC has performed a Phase I Environmental Site Assessment in conformance with the scope and limitations of ASTM Practice E 1527-13 of the property located at 364 & 366 Chestnut Drive, Blowing Rock, NC 28605. Any exceptions to or deletions from this practice are described in Section 2.3 of this report. This assessment has revealed evidence of Recognized Environmental Conditions in connection with the property as summarized in Section 2.3. 6LWHDQG9LFLQLW\'HVFULSWLRQ The Subject Property consists of 3.61 acres and is developed with a 56,050 square-foot, medical complex and a 3,042 square foot, residential building. The property is situated on the northern edge of a ridge and the ground surface slopes to the northwest. Groundcover consists primarily of paved parking areas and commercial landscaping. The subject property can be accessed from the east by paved driveways along Chestnut Drive. The site is zoned HMC for Hospital/Medical Complex. The area surrounding the site is primarily residential properties. &XUUHQW8VHRI3URSHUW\ At the present time, the Property is used by the Blowing Rock Rehabilitation & Davant Extended Care Center. The property card from the Watauga County Tax Administration shows that the medical complex was built between 1954 and 1968. The residential building is listed as being constructed in 1960. 6 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 'HVFULSWLRQRI6WUXFWXUHVDQG2WKHU,PSURYHPHQWV The Watauga County Tax Administration lists the medical complex as two buildings. The initial building is on the east side of the complex and was reportedly constructed in 1954. The larger addition to the west side of the medical complex was completed in 1968. These buildings have a brick exterior siding. The smaller building on the property is listed as a 3,042 residential structure. Several terms will be used in this report to refer to different parts of the medical complex. The initial 1954 building is referred to as the "medical clinic". The 3-story hospital building was constructed in two phases. This 3-story structure will be referred to as "the hospital building". The single story building connecting the medical clinic to the hospital building will be referred to as the connector building. The Town of Blowing Rock supplies drinking water to the Subject Property from the municipal distribution system. Sanitary discharges on the subject site are discharged into the municipal sanitary sewer system. Electricity is provided to the Subject Property by Blue Ridge Electric Membership Corporation. $GMRLQLQJ3URSHUW\,QIRUPDWLRQ During the vicinity reconnaissance, Seramur & Associates, PC observed that adjoining properties are residential. 86(53529,'(',1)250$7,21 6SHFLDOL]HG.QRZOHGJH Seramur & Associates, PC has no specialized knowledge of the Subject Property outside of the research which was conducted and reported as part of this report. The property ownership as well as all individuals who were interviewed as part of this investigation, have not reported any specialized knowledge of the Subject Property outside of what is contained in this report. 9DOXDWLRQ5HGXFWLRQIRU(QYLURQPHQWDO,VVXHV Seramur & Associates, PC has not been provided with an appraisal for the subject property. It is our understanding that this property is to be purchased by Brackett Flagship Properties and this Phase I investigation is part of their due diligence work. No environmental issues were identified by the user/client that could result in property value reduction. 2ZQHU3URSHUW\0DQDJHUDQG2FFXSDQW,QIRUPDWLRQ our interview with Mr. Brett Reece, Plant Operations Manager did not reveal any information which suggested that there are currently or historically any known environmental problems associated with the subject property. 7 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 5HDVRQ)RU3HUIRUPLQJ3KDVH,(6$ The purpose of this Phase I Environmental Site Assessment (ESA) was to identify existing or potential Recognized Environmental Conditions (as defined by ASTM Standard E 1527-13) in connection with the Subject Property. Seramur & Associates, PC understands that the findings of this study will be used to evaluate a pending purchase of the Subject Property. The Phase I ESA is being conducted as part of environmental due diligence prior to property transfer. 5(&25'65(9,(: 6WDQGDUG(QYLURQPHQWDO5HFRUGV6RXUFHV Seramur & Associates, PC contracted Environmental Data Resources, Inc. (EDR) to conduct a search of Federal and State databases containing known and suspected sites of environmental contamination. Ten listed sites identified within the approximate minimum search distance (AMSD) from the Federal and State environmental records database listings specified in ASTM Standard E 1527-13 are summarized in the following table. Detailed information for sites identified within the AMSDs is provided below, along with an opinion about the significance of the listing to the analysis of recognized environmental conditions in connection with the subject property. All ten of the sites listed with a UST or release incident are either down gradient of the property or on the other side of the valley. The one orphan site is listed as a Maple Street address which is also down gradient of the Subject Property. The environmental records search did not identify any listed sites that would effect the environmental quality of the Subject Property. The 2013 USGS topographic map for the site shows the land surface sloping toward the northwest. 10 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 6.3.2 Surface Water Bodies The nearest surface water in the vicinity of the Property is New Years Creek located approximately 1000 feet to the west of the Property. Settling ponds, lagoons, surface impoundment, wetlands and natural catch basins were not observed at the Subject Property during this investigation. 6.3.3 Geology and Hydrology The soil survey maps published by the USDA (Web Soil Survey, 2016) show the Property mapped as Urban Land and is surrounded by the Unaka-Porters Complex which is described as very rocky, cobbly loam over weathered rock. Bedrock is exposed in several areas of the property. The existing structure is likely built on bedrock. Bedrock at the Subject site is the Blowing rock gneiss, an augen gneiss. The depth to groundwater is not known, but the water table is likely below top of bedrock. The water table typically follows the slope of the terrain. Groundwater from the site most likely flows to the northwest toward New Years Creek. No on-site water wells or springs were observed during the Property reconnaissance. Surface water is not present on the subject property. +LVWRULFDO8VH 6.4.1 Historical Summary The purpose of reviewing historical sources is to determine previous uses of the Sbject Property that could have resulted in recognized environmental conditions at the site. Historical information identifying the past site use was obtained from a variety of sources as detailed in Appendix C of this report and included: City Directories, Aerial Photographs, Sanborn Fire Insurance Maps, and/or Topographic Maps. In addition to these sources, persons with knowledge of the Subject Property have been consulted for historical use information. Our knowledge of the history of the property was reported to us by Mr. Reece and Mr. Lewis, employees in the maintenance Department for the last 15 and 18 years, respectively. The property was undeveloped before the early 1950s. The property was developed as a hospital and residence for medical personnel reportedly between 1952 and 1957. The "nurses dorm" is the stand alone residential structure on the property. The medical clinic comprises the eastern portion of the present medical facility. An L-shaped, three-story hospital building was reportedly constructed in 1969. This is the western portion of the medical facility. These two separate buildings were initially connected by an enclosed hallway. The eastern section of three-story building was reportedly added in 1979 and the enclosed hallway was replaced by the one-story connector building between the initial hospital (medical clinic) and the three-story hospital building in 1987. An entrance hall on the southeast corner of the medical complex was reportedly the last addition to the medical complex in 2003. During our walkover of the property, Mr. Lewis showed us an area north of the initial medical clinic and identified it as the ambulance parking/storage area. 11 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 6.4.2 Title Records No title records were provided by the user of this Phase I ESA. 6.4.3 City Directories Business directories including city, cross reference and telephone directories were reviewed by EDR for the following years: 1966, 1992, 1995, 1999, 2003, 2008 and 2013. The EDR City Directory Image Report summarizes the findings of their review. A copy of the EDR City Directory Image Report is included in Appendix C. The Blowing Rock Hospital and the other names for the medical facility were listed in the City Directory for the years 1966 and 2013. The topographic maps show the development in Blowing Rock through the last century. These maps show the appearance of the initial medical clinic in 1959 and the first section of the hospital in 1978. These records show an initial building being constructed in 1954. A second phase of construction is indicated in 1968 and a final phase of development is listed as 1984. Some of the building dimensions listed on the property card can be matched to the existing buildings. (QYLURQPHQWDO/LHQVDQG$FWLYLW\8VH/LPLWDWLRQV No environmental liens or activity/use restrictions in connection with the Subject Property were identified by the user of this Phase I ESA. 6,7(5(&211$,66$1&( 0HWKRGRORJ\DQG/LPLWLQJ&RQGLWLRQV The site reconnaissance was conducted on May 24, 2016 by Keith Seramur and Jacob Schenk of Seramur & Associates, PC. They were escorted through the building and adjacent property by Mr Brett Reece and Mr. Darrel Lewis of the Maintenance Department for the Blowing Rock Hospital. The visual reconnaissance consisted of observing the property around the building. The periphery of the on-site structure was observed along with interior accessible common areas, storage and maintenance areas. Photographs of pertinent site features identified during the site reconnaissance are included in Appendix B. *HQHUDO6LWH6HWWLQJ The property consists of 3.61 acres acres and is developed with a 56,050 square-foot medical complex. The ground surface is level and sloping toward the north-northwest. Groundcover consists primarily of the building, commercial landscaping and asphalt. The subject property is accessed from the east by driveways off Chestnut Drive. 6LWH9LVLW)LQGLQJV 7.3.1 Hazardous Substances Seramur & Associates, PC found evidence of facility maintenance materials some of which have hazardous properties. For the most part these materials are contained in gallon containers a/or smaller. Hydraulic oil is used in several pieces of equipment throughout the facility. Fuels, motor oil and related chemicals are used in the maintenance ares. Although some stained concrete was observed in different areas of the facility, we did not observe evidence of a release of these materials into the environment. Reportedly non-toxic, non-corrosive andbiodegradable 7.3.2 Petroleum Products Seramur & Associates, PC found evidence of petroleum products on the property. The miscellanous containers of oil and gasoline were observed in the maintenance rooms in the center of the complex. Although stained concrete was observed in some storage areas there was no evidence of a release to the environment. 7.3.3 USTs The Subject property contains one 10,000 gallon heating oil UST that is in use. This UST is registered with the NCDEQ and the Subject Property has a Facility ID number of 29682. The tank is listed in the NCDEQ data base as being installed in 1964. Mr. Reece indicated that the tank had been re-lined in the late 1980s. We observed the fill and vent pipes for the UST on the south side of the hospital building. A pool of fuel was observed in the catch basin or spill bucket around the fill pipe. This tank reportedly fuels the three boilers at 15 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 the facility and is reportedly hooked up to the emergency generator. Fuel lines to the boilers ran along the ceiling of the boiler room. Two copper pipes stuck out of the concrete slab next to each of the boilers. These pipes were cut off a couple of inches above the floor and were capped. Mr. Lewis informed us that these capped pipes were the old fuel lines from the UST. There is a UST along the north side of the medical complex that is used to store grease residuals from the kitchen. A large grate and manhole was observed north of the three story hospital building. Mr. Lewis reported that this is a large tank designed to retain storm water onsite and slowly discharge it to nearby surface water drains. Recognized Environmental Conditions The pool of fuel in the spill bucket around the UST shows that overfilling the fuel while filling the UST is not uncommon. Spill buckets have been in use for the last 10-20 years. These buckets can also overfill and leak into soil around the UST. Prior to 1988 spill buckets were not required and fuel spills while filling USTs typically drianed into the soil around the UST. The UST has been in the ground for over 50 years. The elevated topography of the Subject Property would produce a well drained soil and limit the amount of soil moisture around a steel tank. The tank was lined (typically with fiberglass) in the late 1980s. The effectiveness of these linings has been questioned. There is a reasonable possibility that the integrity of a steel UST that has been in the ground more than 50 years has been compromised. The older fuel lines in the floor of the boiler room were abandonned for a reason. This could have been done during the USTsystem upgrade in the late 1980s. It is also possible that the fuel lines were replaced after an unexplained loss of fuel from the tank. The initial medical clinic was constructed in the mid-1950s. After 1964, this part of the complex was heated with boilers that were fueled from the 10,000 gallon UST. It is not known how the medical clinic was heated between the years of 1954 and 1964. The medical clinic also had an incinerator. The door to the old incinerator is still attached to a brick wall in the building although the incinerator components have been removed. Mr. Reece stated that he did not know what fuel was used to fire the incinerator. Heating oil would have been a likely source of heat for the medical clinic between 1954 and 1964. We did not observe fill or vent pipes from a heating oil UST around the outside of the medical complex. It is possible that an old heating oil UST remains in the ground around the former medical clinic. 7.3.4 ASTs Two propane ASTs were observed on the northwest side of the medical complex near the kitchen. There are 2 hydraulic oil ASTs in the mechanical rooms for the elevators. There is also a tank with treated boiler water. 16 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 Mr. Lewis reported that the residential structure was heated by an above ground storage tank (AST) located behind a bush on the south side of the structure. He showed where the bush was located and said that they had planted the bush to cover the AST. There is an AST on the northwest corner of the medical complex that is used to store used cooking oil. Mr. Reece also informed us that there is a salt brine AST used to treat the parking areas for ice. Recognized Environmental Conditions Soil quality around the former AST could have been affected by spillage from this tank over the years. Stained soil was not observed during our reconnaissance. 7.3.5 Other Suspect Containers Evidence of other suspect containers was not observed on the subject property during the site reconnaissance. 7.3.6 Equipment Likely to Contain PCBs According to the U.S. EPA, "PCBs were domestically manufactured from 1929 until their manufacture was banned in 1979. Although no longer commercially produced in the United States, PCBs may be present in products and materials produced before the 1979 PCB ban. Products that may contain PCBs include: • Transformers and capacitors • Other electrical equipment including voltage regulators, switches, reclosers, bushings, and electromagnets • Oil used in motors and hydraulic systems • Old electrical devices or appliances containing PCB capacitors • Fluorescent light ballasts • Cable insulation • Thermal insulation material including fiberglass, felt, foam, and cork • Adhesives and tapes • Oil-based paint • Caulking • Plastics • Carbonless copy paper • Floor finish" Based on the estimated construction date of the structure on the property, PCBs could be contained within products and materials present at the site. Mr. Reece reported that the original transformer for the medical clinic was inside the building. He stated that BREMCO (the electricity supplier) had removed that transformer in the late 1970s and replaced it with a PCB-free transformers outside of the building. 17 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 7.3.7 Interior Staining/Corrosion Interior staining or corrosion was observed in the subject building during the site reconnaissance. Floor drains were present in the 3 mechanical rooms and in the boiler rooms. Staining around the floor drains in the boiler room appeared to be a white precipitate. We were informed that this was from boiler water discharging to some of the drains. Compressor "blow down" (condensate from the compressor tank) can contain compressor oil. Some compressors in the medical complex were set up so the blown down discharged to the floor drains. We were informed that the floor drains discharged to the sanitary sewer. 7.3.8 Discharge Features Discharge features observed on the subject property during the site reconnaissance were mainly storm water drains designed to handle storm water run off from the roofs, enclosed lawns and parking areas. There is a compressor in one of the courtyards for the medical clinic that discharges onto the lawn. A 1.5-inch PVC pipe was observed protruding from beneath the stairs at the west end of the medical complex. This stairs is adjacent to the loading dock. Mr. Lewis stated that this was draining a wet-weather spring located under that side of the building. Recognized Environmental Condition The compressor blown down discharging onto the lawn in a courtyard for the medical clinic could have discharged compressor oil into the soil. 7.3.9 Pits, Ponds, And Lagoons No pits, ponds or lagoons were observed on the Subject Property during the site reconnaissance. 7.3.10 Solid Waste Dumping/Landfills No readily apparent evidence of solid waste dumping, suspect fill material, or landfills was identified on the Subject Property during the site reconnaissance. 7.3.11 Stained Soil/Stressed Vegetation Stained soil and stressed vegetation were not observed on the Subject Property during the site reconnaissance. 7.3.12 Wells No evidence of water supply or groundwater monitoring wells was observed on the Subject Property during the site reconnaissance. 18 Phase I Environmental Site Assessment Report Blowing Rock Hospital May 31, 2016 8.0 INTERVIEWS The purpose of the interviews is to obtain information about past and present uses of the Subject Property and their potential impact on the environmental condition of the Property. ASTM also requires that the owner(s), or their representative, be asked whether the following documents are available for the Subject Property: environmental site assessment reports, registrations for underground or above ground storage tanks, reports of hydrogeological conditions, governmental notices or correspondence relating to environmental violations or environmental liens, and geotechnical studies. The Key Site Manager was not aware of any past environmental reports for the facility. Interview with Key Site Manager Our knowledge of the history of the property was reported to us by Mr. Brett Reece, Plant Operations Manager who has worked at the Blowing Rock Hospital for 15 years. Mr. Darrel Lewis an employee in the maintenance Department for the last 18 years also provided us much insght into the history of the Subject Property. Mr. Reece is considered the Key Site Manager for the purpose of this Phase I ESA. Information provided by Mr. Reece and Mr. Lewis is discussed above. Mr. Reece provided responses to questions concerning on the Phase I ESA Questionnaire. This Questionaire is included in the Additional Documentation (Appendix E). Interview with Local Government Official On June 6, 2015, Seramur & Associates spoke with the Town of Blowing Rock Fire Department personnel about possible past emergency response events at the Blowing Rock Hospital. Deputy Fire Chief, Mr. Matt McGuire was interviewed about the emergency response calls and spills at the Blowing Rock Hospital. Mr. McGuire has worked for the Blowing Rock Fire Department for the past 15 years. Mr. McGuire stated that they have no record of spills or hazardous material releases at the property. He indicated that their emergency response records are reviewed for a five year period. FINDINGS We have performed a Phase I Environmental Site Assessment in general conformance with the scope and limitations of ASTM Standard Practice E 1527-13 of the Blowing Rock Hospital at 364&366 Chestnut Drive, Blowing Rock, Watauga County, North Carolina, the Subject Property. Any exceptions to, or deletions from, this practice are described in Sections 3.4 and 3.5 of this report. Spill buckets have been in use for the last 10-20 years. These buckets can also overfill and leak into soil around the UST. Prior to 1988 spill buckets were not required and fuel spills while filling USTs typically drianed into the soil around the UST. • The UST has been in the ground for over 50 years. The elevated topography of the Subject Property would produce a well drained soil and limit the amount of soil moisture around a steel tank. The tank was lined (typically with fiberglass) in the late 1980s. The effectiveness of these linings has been questioned. There is a reasonable possibility that the integrity of a steel UST that has been buried for more than 50 years old has been compromised. • The older fuel lines in the floor of the boiler room were abandonned for a reason. This could have been done during the UST system upgrade in the late 1980s. It is also possible that the fuel lines were replaced after an unexplained loss of fuel from the tank. 2Q6LWH)LQGLQJVASTs • Soil quality around the area of the former heating oil AST at the residential structure could have been affected by spillage from this tank over the years. 2Q6LWH)LQGLQJVHistorical Use • The initial medical clinic was constructed in the mid-1950s. After 1964, this part of the complex was heated with boilers that were fueled from the 10,000 gallon UST. It is not known how the medical clinic was heated between the years of 1954 and 1964. The medical clinic also had an incinerator. Mr. Reece stated that he did not know what fuel was used to fire the incinerator. Heating oil would have been a likely source of heat for the medical clinic between 1954 and 1964. It is possible that an old heating oil UST remains in the ground around the former medical clinic. Off-Site Findings None This section concludes the findings of the Phase I ESA. This Phase I ESA process is intended to reduce, but not eliminate uncertainty regarding the potential for environmental conditions.
