Mr. Heath,

The Division of Waste Management, Solid Waste Section (Section) has received and reviewed the
Construction Quality Assurance Report (DIN 26831) for Phase 2B-2 at the above referenced
facility. The report was submitted on your behalf by Smith Gardner, Inc. and was received in the
Central Office on September 28, 2016. The report was forwarded to the Asheville Regional Office
on October 10, 2016.

Based on our review, it appears the pre-operative conditions have been met so operation of Phase
2B-2 is authorized. Additionally, the Closure, Post-closure and Potential Assessment and
Corrective Action cost estimates, totaling $3,294,910, are approved. The financial assurance
mechanism for the facility must be updated to include these new costs and submitted to the Section
as soon as possible. Operation of Phase 2B-2 must be managed in accordance with currently
approved operations plan. The Permit will be updated to include Phase 2B-2, and any associated
documents, once the Phase 3 Permit to Construct Application review is complete.
