Comprehensive Facility Compliance Inspection.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1 - The Permit and Ops Plan were available for review, and were reviewed.
2 - The facility takes in, on average, 450-500 tons per day. The facility is permitted to receive as much as 1000TPD.
3 - The service area includes the following counties: Duplin, Edgecombe, Frankin, Greene, Halifax, Johnston, Lenoir,
Martin, Nash, Northampton, Pitt, Wake, Wayne, and Wilson Counties. Waste must be transported for disposal to either
Sampson Co. Permit No. 82-02 or Republic Services Bertie Permit No. 08-03.
4 - The operating hours are Monday thru Friday 7:00 AM to 4:00 PM.
5 – Facility has proper signage with required information per permit conditions.
6 - Waste screening is being conducted periodically and records kept in available 3 ring binder, screening records were up
to date and good. The Ops Plan specifies that each load be inspected by a spotter.
7 - Fire control is managed by two charged fire extinguishers and has a 2” hose bib in the transfer building.
8 – Facility Certified Operators: Ricky Knight, expires March 24, 2018. Pamela E. Price, expires November 13, 2018.
Michael R. Lee, expires July 16, 2017. H.A. Austin, expires December 06, 2016. Facility has additional certified operators
available if needed.
9 – The tipping floor was clean and drainage was working like it should. The new pipe work appears to be functioning
good to prevent the prior clogging issues.as mentioned in prior inspection reports.
10- Facility has purchased cow waterers to place behind trailers to catch any potential leaks from trailers.
11 – Windblown trash appears to be managed very good per the Inspection today.
12 – Surface water swell area has been corrected with rip rap to prevent erosion and if functioning well.
13 – Empty trailers are stored in front of the tipping floor. Trailer number 1217 (holes) and 3434 (door seal) are in need of
repair as to prevent leakage.
14 – Full trailers are stored on concrete pad behind the tipping floor. Trailer number 7910 (holes) is in need of repair as to
prevent leakage.
15 – Facility contracts with CEI for transportation of the MSW to permitted landfills listed above. CEI was notified as to
the trailer issues.
16 – Digital pictures taken during this inspection.
