Box 2197 Apex, NC 27502     PARTICIPANTS: Michelle Sclafani, NCDENR/DWM/SWS Josh McMahan – Elite Waste Services     STATUS OF PERMIT: New Firm     PURPOSE OF SITE VISIT: New Truck Inspection   STATUS OF PAST NOTED VIOLATIONS: N/A.   OBSERVED VIOLATIONS: None.         The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste coll ection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.           Page 2 of 2 FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section   ADDITONAL COMMENTS   1. Inspected pump truck VIN #1FDUF5GY3FED71914, and license tag FD-6435 with tank capacity of 950 gallons. 2. Vehicle was lettered with correct company name, location, phone number, and NCS ID on both sides of the cab of the truck. The lettering of the name measured 3.25 inches, and the other letters were at the three inch minimum. 3. The tank was structurally sound, securely bolted to the frame of the truck and constructed out of approved materials. 4. Valves and access ports present and in proper working order. Caps were present and secured to access ports to prevent leaks. 5. No evidence of any leaks from the tank, valves, or access ports. 6. Records were not inspected as this is a new truck. No evidence that truck has been in use before being permitted. 7. No issues with the truck. Truck passed inspection and can be used.
