Dear Mr. Lamb:

After completing a review of the Work Plan for the Leak Location Test at the Great Oak Landfill -
Cell1A dated September 12, 2016 and revised September 22, 2016 (DIN 26805); the Solid Waste Section
(SWS) hereby approves the above-referenced Work Plan. Please be aware that this approval does not
release the current or future permit holder from talking regulatory corrective measures for leachate
releases due to the liner damage or operation of waste disposal activities. The SWS will require the
Environmental Senior Specialists and or staffs be allowed to view throughout the testing events and
request that your contractor – LLSI provides a brief description of testing procedures. Please inform

[e-mail] as soon as possible when the firm date of the testing events is scheduled. Your
cooperation on this matter is highly appreciated.
