The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Took soil samples, received check for NCDA. Reviewed logs – the field ID is missing on the logs sheets (this
was noted earlier this year & also on the audit of 10/16/2015). The operator shall review the logs from
August 2014 (when the last permit was signed) until now & ensure that the field ID (field 1 or 2) is clearly
noted for all discharge events on the land application site logs. This was discussed with Mr. Locklear.
2. Field markers appear accurate however on field 2 there is a marker completely hidden by vegetation & another
slightly obscured. Discussed with Mr. Locklear that the vegetation shall be completely removed from around
both markers so that they are visible and that the 10’ buffer between the neighboring vegetation & the
disposal fields shall be maintained.
3. It appears that the approved setbacks are being met. No sign of application outside the permitted boundaries.
4. There is an old hay bale sitting in the buffer zone between field 1 & the railroad that needs to be removed.
5. There are sparse areas of Bermuda – possibly due to heavy application (noted areas of matting). Discussed with
Mr. Locklear that adequate truck speed, pressure and spreader application are needed to ensure a wide
thin spray path & that matted areas may be raked loose to prevent crop damage. Also noted some
weeds/vegetation other than Bermuda.
6. The hay has been cut on field 2. Establish the winter cover crop as per the approved nutrient management
plan so that land application may occur this winter.
7. No recent application and no odor noted.
8. The required signs are posted and the access road is adequate at this time.
