The permit shall expire July 30, 2018. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Proper signage was observed at the entrance of the facility. 2. The facility is permitted to receive solid waste generated within Iredell County. 3. The facility accepts an average 728.65 tons per day. Operations Inspection of the MSW Landfill Unit: 4. The facility was currently working in Phase 5. 5. No unacceptable waste streams were observed in the MSWLF. A tire had been pulled out and set aside to be moved to the tire collection area. 6. Edge of liner markers were observed. 7. All access roads were in good condition. 8. Windblown waste was being collected during the inspection. 9. A small area of waste was observed with inadequate Posi-Shell application. The proper use and application of Posi-Shell was discussed with Landfill staff. Mr. Beam said the compactors had run over the area and they would be applying Posi-Shell over the entire working face including that area before the end of day. Mr. Beam also stated that by the end of the day on Wednesday September 7, 2016 the entire area would be covered with dirt. Operations Inspection of the Closed C & D Landfill Unit: 10. The CDLF discontinued acceptance of waste on June 27 2008. 11. The post closure period for the CDLF began on May 8, 2013. 12. A good stand of vegetation was observed on the CDLF. 13. Edge of liner markers were observed. Operations inspection of the Bailer Facility: 14. The Iredell County Baler Facility (Permit # 49-03-TP) has been incorporated into the Iredell County MSWLF Permit # 49-03. 15. The baler facility is used by the general public and small generators as a drop off area. Waste is dumped onto the tipping floor and loaded into roll-off boxes to be hauled to the MSWLF. 16. Only recyclable material is baled at the facility. 17. Periodic random inspections are conducted inside the baler facility. Operations Inspection of the White Goods Collection Area: 18. This area was well managed at the time of inspection. 19. White goods containing Freon are separated from other metals and stored in an upright manner. 20. Freon is extracted by Landfill staff. Operations Inspection of the Electronic and Paint Collection Area: 21. This area was well managed at the time of the inspection. 22. Two cans of concrete sealer were leaking and pointed out to staff. They immediately moved the leaking cans to a container. 23. Latex paint collected in five (5) gallon containers is mixed with the Posi-Shell. Operations Inspection of the Tire Collection Area: 24. This area was well managed at the time of inspection. 25. All tires were being collected in covered trailers and were being stored properly. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 3 Operation Inspection of Mulching and Grinding / Yard Waste / Type I Small Composting Area: 26. This area was well managed at the time of the inspection. 27. Land clearing waste and pallets are kept separated from yard waste that is required to be composted. 28. One pallet with paint, OSB and the pallets with the feet were pointed out. Mr. Lambert radioed the operator to pull the items out of the pile. Landfill staff stated all mulch ground from pallets is used to maintain roadways and not distributed to the public. 29. Yard waste compost temperature records were reviewed and are not being maintained in accordance with the rules. Temperatures were recorded for three (3) consecutive months. Yard waste is in process and is not being distributed. The proper record keeping was discussed with staff and from this point forward temperatures shall be maintained at or above 131 degrees for three (3) consecutive days to ensure that the pathogen reduction criteria is met in accordance with 15A North Carolina Administrative Code 13B .1406(10). Operations Inspection of the Asphalt Shingle Recycling Area: 30. The asphalt shingle recycling area was well managed at the time of the inspection. 31. No unacceptable waste was observed. Records Review: 32. Reviewed permit, operations plan, training records, semi-annual groundwater, surface water and leachate monitoring reports (January 2016), quarterly methane monitoring reports, random inspection records (2-3 per week), waste determination records, leachate lagoon inspections, leachate discharge records (538,500 gallons discharged August 2016), scrap tire certification records (177,974 tires disposed 2015-2016), Shingle recycling records (tonnage and asbestos sampling). 33. During the inspection while reviewing the surface water and leachate monitoring reports the facility is flushing the leachate lines once per year. The leachate lines are videoed every five (5) years. Leachate collection maintenance Phase 1-4 was last performed on August 2012; Phase 5 expansion was last performed on June 2014. 34.
