Dear Mr. Mitchell:
Enclosed you will find a copy of the Alexander County Permit No.’s 0201 and 0202, dated
August 30, 2016, DIN 26659. The Permit to Operate has been amended, five (5) year renewal,
extending the permit expiration dates to November 2020 and December 2020 respectively.
Please note the following;
 The facility permit is issued for a five (5) year period since the rules for life of site
permitting are still pending.
 Read your permit carefully, it is important that you understand your permit. If you have
any questions, contact me immediately.
 Copies of this permit, the approved plans, and all records, are required to be maintained
by the permittee at the facility, and made available to the Section upon request during
normal business hours.
