OBSERVED VIOLATIONS:
1. Rule .0838(a)(15) Grease septage shall be diluted at least 1:1 from its concentration when pumped with domestic
septage or water if land applied over perennial vegetation. This dilution shall be increased if crop damages occurs.
It appears that a grease burn is present on Field 1. Take measures to ensure that all grease septage is adequately
diluted to prevent crop damage. Repair this burned strip as per the approved nutrient management plan.
2. .0838(a)(18) Approved nutrient management plans shall be followed.
Hay bales are sitting on both fields and appear to have been there for some time since harvest due to vegetation
that has grown up. It appears that land application has occurred with the hay bales sitting on the fields.
Per the nutrient management plan there shall be a 30 day wait prior to harvest with the harvested crop removed for
cattle feed. Land application shall not occur until the harvested crop is removed.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Remove excess vegetation from around the posted sign at the site entrance so that it is clearly visible.
2. Field markers appear adequate based on the current site map, however unsure at this time about the dividing
line between field 1 & field 2.
3. It appears that approved setbacks are being met, but setbacks were not measured at this time.
4. Septage discharges are within the permitted boundaries.
5. No heavy application or spot dumping noted, however there does appear to be a grease burn.
6. Crop stand appears adequate. Control weeds & vegetation other than Bermuda. Establish the cover crop this fall
per the approved nutrient management plan so that land application may occur this winter.
