Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. The required sign is posted and the access road is adequate. 2. It appears that the approved setbacks are being met, but setbacks were not measured at this time. 3. It appears that septage application are occurring within the permitted boundaries. 4. No significant odors noted. 5. Field markers appear accurate except one may be missing at the back 100’ corner marker – see enclosed map. 6. Ms. Keen stated that the field had just been sprayed for weeds (lack of groundcover on the disposal field at this time) and that soybeans would be planted very soon. Per the permit & nutrient management plan soybeans to be planted in June or July. 7. Signs of recent application. Per the nutrient management plan – (for soybeans) septage to be land applied from preplant until they reach 3 to 6 inches tall – (for rye) septage to be land applied 7 to 10 days before planting and during springtime. For land application to occur on this site the nutrient management plan shall be followed in regards to crop establishment. Please ensure the monthly application rates (low, medium, high or none) are being followed. 8. Ms. Keen stated that possibly the approved crop on this site would be changed to Bermuda/rye. A revised nutrient management plan written by a certified technical specialist shall be submitted for this crop change. Also please submit the enclosed application form. The new cropstand shall be adequately established and maintained for land application to occur. A new permit shall be issued and approval granted by the Division before land application commences on the new cropstand. Please inform this office of whether the approved crop will change – need to know in regards to the renewal of this SLAS permit. 9. For permit renewal please submit a copy of the site logs from August 2010 until now.
