with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Unable to access site due to major construction between the state road and the site. Ms. Keen stated that turkey
houses were being constructed in this area and that land application had stopped in January. Ms. Keen is in the
process of finding out if the land application site has to be closed due to the turkey operation.
2. Enclosed is a cease operation form. If you wish for this SLAS permit to be closed out please submit this
form. Please submit a copy of your site logs for September 2014 until land application ceased early this year.
The last soil analysis & the site logs will be reviewed to determine if another soil sample is required. Per Rule
.0843(e) Prior to final closure, the soil pH of the site shall be raised to 6.5 unless fertility requirements for crops
to be grown in the following year dictate less. Based on the final soil pH additional lime may need to be added
to the disposal fields.
3. If you wish to renew the permit for this site & for land application to occur in the future please contact
me in regards to other items that are needed for the permit renewal.
