The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Site visit in regards to issuing permit under new permit holder. Verified required setbacks & GPS’d fields for
new site map. On Field 1 a 50’ setback is required on the west side (two trees will be marked as corner posts), a
50’ setback is required on the south side (a row of mature pines designate this line) and a 50’ setback is required
along a portion of the front east side (placed two orange flags at each corner to delineate this front line – please
replace with permanent posts). The storage tank currently sitting at the front corner of Field 1 shall be moved
out of the disposal area. On Field 2 the required 100’ setback from neighboring ponds was measured (the fence
line on this north west side meets the setback), a fence on the lower south side meets the required 50’ setback
here, a fence on the east end cuts the field off at this point and soil was checked in the middle of this field to
establish the boundary. Discussed that certain fence posts (that designated disposal boundaries) & all orange
flags shall be replaced with permanent posts on Field 2. Various pieces of equipment stored along the fence
lines of field 2 need to be removed.
2. Chester reviewed permit submittal with John Long.
3. No septage land application or storage occurring at this site or permitted at this time.
