This permit shall expire on November 5, 2019. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The facility is permitted to receive Household Hazardous Waste (HHW) from Ashe County only. 2. The facility also acts as a convenience center for the collection of electronics, used oil, oil filters, vehicle batteries, cardboard, aluminum cans, glass bottles, newspapers, plastic bottles and scrap tires. 3. The permit and operations plan were verified. Both documents were current and available upon request. 4. The facility provides annual training for employees. These documents are kept at the facility and made available upon request. 5. A gate was provided at the entrance of the facility to prevent unauthorized access. The facility is also surrounded by a chain-length-fence. 6. The facility had proper signage. 7. Facility emergency and safety equipment is inspected daily. Records for January 2016 through the date of the inspection were verified. Equipment checked includes gloves, hard hat, aprons, mask and cartridges, and the eye wash station. 8. A Customer Registration Form is completed for each load of waste received at the facility. These forms verify the type and amount of waste received, and that the waste is generated from a household within Ashe County requiring the customer’s signature. Forms for January 2016 through the date of the inspection were verified. 9. All HHW is stored in a steel, trailer-type structure with three separate compartments. The first compartment (left to right) is designated for storage of oil based paint and flammable liquids. The second compartment is designated for storage of latex paint, and the third compartment is designated for storage of insecticides, herbicides, and antifreeze. All compartments were well maintained. 10. All HHW storage compartments were equipped with a secondary containment area beneath the floor to prevent spills from being lost. 11. All sump outlets leading from secondary containment areas were capped at the time of inspection. 12. All observed drums were clearly labeled with the date each period of accumulation began as required. 13. Absorbent material was present on site for clean-up of spills or leaks of small quantities. 14. The HHW storage building was equipped with a dry chemical suppression system. Mr. Houck confirmed in a phone conversation on July 28, 2016 that the fire suppression system was last serviced on March 12, 2015. He stated that he planned to notify the service contractor to inform them that the yearly service of the device was overdue. 15. Pad locks were provided for each HHW compartment. 16. A metal building is provided on site for the collection of florescent bulbs. All bulbs were packaged for shipment to the vendor at the time of inspection. Florescent bulbs are sent to Southeast Recycling Technologies, Inc. out of Johnson City, Tennessee. 17. Access around the facility was free of obstacles. 18. Mr. Houck confirmed by electronic correspondence on July 28, 2016 that air horns were provided at the facility as requested on the previous inspection report. Mr. Houck provided a photo of the purchased air horns. Thank you for addressing this concern. 19. Waste manifests were verified for pickups performed on 9/1/15, 11/2/15, 1/5/16, 3/22/16, and 6/7/16. ECOFLO, Inc. is contracted for pickup of HHW from the facility. 20. Pickup of herbicides and pesticides from the facility is coordinated by the NCDA & CS. Since the last inspection, ECOFLO, Inc. has been contracted to collect the subject materials. 21. Herbicides and pesticides were last collected from the facility on 4/16/15 and 6/2/16.
