OBSERVED VIOLATIONS:
1. 15A NCAC 13B .0838 (a) (18) states: “Approved nutrient management plans shall be followed”.
A) The approved nutrient management plan states: “A 30-day minimum waiting period will be observed after
septage applications before grazing and/or forage harvesting occurs.”
Page’s Septic Tank Cleaning was in violation of 15A NCAC 13B .0838 (a) (18) because cattle were observed
on Field B which was actively being used for septage application, therefore, the approved Nutrient
Management Plan was not being followed.
In order to achieve compliance, Page’s Septic Tank Cleaning must:

a. Immediately restrict cattle from grazing on Field B, and
b. Submit a written plan describing actions that will be taken to prevent cattle grazing on a septage land
application field while it is being actively used. Please submit this plan by February 19, 2016 to:

B) The approved nutrient management plan also states: “Re-seeding and/or inter-seeding will be conducted on
an as needed basis when stand density falls below 80%. Page’s Septic Tank Cleaning was in violation of 15A
NCAC 13B .0838 (a) (18) because the tall fescue and/or orchard grass was less than 80% of Field C,
therefore, the approved Nutrient Management Plan was not being followed.
In order to achieve compliance, Page’s Septic Tank Cleaning must:
a. Follow the approved management plan for re-seeding and/or inter-seeding. The dates for this activity is
listed in the plan as “between February 15 and March 31”, and
b. You may also contact the Davie County Cooperative Extension at 336-753-6100, for advice on field
maintenance and weed control.
ADDITIONAL COMMENTS
1. All photographs were taken by Troy Harrison on November 17, 2015.
2. The Septage Land Application Site consists of Field B (5.7 acres) and Field C (5.1 acres) for a total of 10.8
usable acres.
3. The site is permitted for land application of domestic septage (including portable toilet waste) and grease
septage.
4. The site had accessible all-weather roads to the septage land application field covered under the permit.
5. Appropriate signage was displayed at the site.
6. All fields had disposal area boundaries that were well delineated with permanent markers which corresponded
to the site map.
7. Field B was has recent land applications however cattle was observed grazing in this field. It also
appeared that the vegetation on this field is routinely grazed. The cattle were moved into an adjacent
field (not Field C) during the inspection and the gate closed. The Nutrient Management Plan (Plan)
requires a 30-day minimum waiting period to be observed after septage applications before grazing
and/or foraging harvesting occurs. This field is approved for the planting of tall fescue and orchard
grass and requires re-seeding or inter-seeding when the stand density falls below 80%. It appears that
this field may be approaching the 80% limitation. Fields cannot be used for land application unless an
adequate tall fescue and orchard grass crop is established and maintained.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

8. Field C was currently not accepting septage land applications and had grazing cattle. This field is
approved for the planting of tall fescue and orchard grass and requires re-seeding or inter-seeding when
the stand density falls below 80%. It appears that this field is past the 80% limitation and needs re-
seeding. Fields cannot be used for land application unless an adequate tall fescue and orchard grass
crop is established and maintained.
You may also contact the Davie County Cooperative Extension at 336-753-6100 for advice on field
maintenance and weed control.

9. No evidence of standing liquid or pooled waste was observed.
10. No objectionable odors were observed.
11. The nutrient management plan appears to have been followed on all fields.
12. Site logs were not reviewed at this time.
13. Soil samples were collected from the field. The check for soil sampling was received.
