Enclosed you will find a copy of Permit No. 3615-TP-2012, dated August 17, 2016, DIN
26598. The Permit to Operate has been amended, five (5) year renewal, extending the permit
expiration date to December 12, 2021 for the Assured Waste Solutions LLC, Medical Waste
Treatment Facility, located in Gaston County. Please note the following;

 The approved document related to this permit is attached.
 Copies of this permit, the approved plans, and all records, are required to be maintained
by the permittee at the facility, and made available to the Section upon request during
normal business hours.
